Back pain is one of the most common reasons people walk through our doors at Cameron Family Chiropractic. But here’s the problem:
Not all back pain is the same.
Sometimes it’s muscular — tight, achy, stiff. Other times, it’s disc or nerve-related — sharp, burning, shooting, radiating. And if you mistake one for the other, you’ll end up with the wrong treatment… and often more pain down the road.
I’m Dr. Chris Cameron, and today I want to help you understand the key differences between muscular back pain and disc-related issues, why misdiagnosis is so common, and what we do differently to get patients the right answers.
Muscular Back Pain: The “Tight and Achy” Type
Muscular back pain usually feels:
- Tight, stiff, or achy
- Localized (stays in one spot, doesn’t shoot down the leg)
- Better with movement, stretching, or light exercise
- Worse with inactivity or stress
- Relieved by heat, massage, or mobility work
Muscle pain is often caused by overuse, poor posture, or minor strains. It’s uncomfortable and annoying, but in most cases, it’s not dangerous.
Disc or Nerve-Related Pain: The “Shooting, Radiating” Type
Disc or nerve pain, on the other hand, is a different story. It often shows up as:
- Sharp, burning, or shooting pain down the leg (sciatica)
- Worse with sitting, bending, or coughing
- Numbness, tingling, or weakness in the leg or foot
- Pain that does not improve with stretching or massage
- Deep, intense discomfort that feels different from muscle pain
This type of pain is often caused by a bulging or herniated disc pressing on a nerve. When that’s the case, the priority is reducing the pressure, calming the inflammation, and stabilizing the spine — not just stretching or massaging.
Why MRIs Don’t Always Help
A lot of people ask, “Can’t I just get an MRI to find out?”
The truth: MRIs show structure, not function.
- Plenty of people have disc bulges on scans — and no pain.
- Others have clean scans — but terrible back pain.
That’s why imaging alone isn’t enough. At our clinic, we combine imaging (when appropriate) with functional testing:
- Movement screens (flexion, extension, rotation, weight-bearing positions)
- Neurological exams (reflexes, strength, coordination)
- Muscle activation testing (especially stabilizers that protect the spine)
This approach helps us find out why it hurts — not just where.
What Happens if You Treat the Wrong Problem?
It’s more common than you think.
If you treat disc pain like muscle tightness, you might feel a little better for a few days. But the pressure and inflammation continue — and the pain comes back, sometimes worse.
I’ve seen patients spend months foam rolling, stretching, and doing yoga for “tight muscles” only to find out the real issue was a disc pressing on a nerve. Once we corrected the diagnosis and applied the right treatment, everything changed.
Real Patient Story
One of our patients, let’s call him John, thought he had stubborn muscle knots in his glute. He stretched, did yoga, rolled on a ball — but nothing worked. The pain shot down his leg, but he assumed it was just “tight muscles.”
When he came in, we ran a few neurological tests and found weakness in his ankle and reduced reflexes — classic signs of sciatic nerve irritation from a disc problem.
Within 6–8 weeks of targeted decompression, laser therapy, and movement retraining, the shooting pain was gone. And most importantly, it stayed gone.
How We Figure It Out
At Cameron Family Chiropractic, we don’t guess. We test.
During a BackStrong Evaluation, we look at:
- How your spine moves
- How your nerves are functioning
- How your stabilizer muscles are firing
- Your posture, alignment, and movement patterns
From there, we build a plan that matches the cause:
- If it’s muscular → hands-on care, mobility work, rehab
- If it’s disc-related → decompression, laser therapy, anti-inflammatory support, and core stability training
Every plan is customized to your body and your pain.
The Bottom Line
Back pain isn’t “just” back pain. Getting the diagnosis right is the key to real healing.
If you’ve been stuck treating your back pain the same way over and over — with no results — it might not be the treatment that’s wrong. It might be the diagnosis.
