Back Pain vs. Nerve Pain: Why the Difference Matters

Back Pain vs. Nerve Pain: Why the Difference Matters

If you’ve been dealing with back pain that just won’t go away — especially if it travels down your leg, feels sharp or electric, or comes with numbness or tingling — it’s time to ask one key question:

Is this really back pain… or is it nerve pain?

The answer matters because muscle pain and nerve pain need very different approaches. Treating them the same way is one of the biggest reasons people stay stuck in a cycle of flare-ups and frustration.

Hi, I’m Dr. Chris Cameron from Cameron Family Chiropractic. Today I want to explain the difference between mechanical back pain and nerve-related pain, why the wrong diagnosis can lead to the wrong care, and how we help patients get the right answers — and lasting relief.

Mechanical Back Pain: The Common Type

Mechanical back pain is the most common type we see. It usually comes from muscles, joints, or discs and tends to stay local to the lower back.

Common signs include:

  • Aches, stiffness, or soreness in the back
  • Pain that stays in one spot instead of traveling
  • Relief with movement or stretching
  • Worsening with inactivity or poor posture
  • Responds well to heat, massage, or gentle exercise

While uncomfortable, this type of pain is rarely dangerous and often improves with activity and basic care.

Nerve-Related Pain: A Whole Different Experience

Nerve pain looks and feels very different. It often comes from a disc bulge or herniation pressing on a nerve root, or inflammation around the nerve.

Signs of nerve-related back pain include:

  • Sharp, shooting, burning, or electric pain
  • Pain that travels down one leg (classic sciatica)
  • Numbness, tingling, or weakness
  • Worsening with sitting, bending, coughing, or straining
  • Does not respond well to stretching or massage

Unlike muscle pain, nerve pain doesn’t settle down on its own. In fact, pushing through it with the wrong care can make it worse.

Why the Right Diagnosis Matters

Here’s what we see all the time: someone has been treating nerve pain like it’s a tight muscle. They’ve tried stretching, massage, yoga, even core work — but the pain keeps coming back.

That’s because nerves heal differently.

  • Muscles have a rich blood supply and heal quickly.
  • Nerves take longer, and they hate being compressed or inflamed.

The longer a nerve is irritated, the more sensitive it becomes and the harder it is to heal. That’s why it’s so important to figure out the real source of pain as early as possible.

How We Test for Nerve Pain

At our clinic, we don’t guess — we test. Our BackStrong Evaluation includes:

  • Orthopedic exams to see how the spine moves
  • Neurological screenings for reflexes, strength, and sensation
  • Functional testing to assess posture, balance, and movement

Often, the signs of nerve involvement are clear even before advanced imaging like an MRI is needed.

Our 3 Core Tools for Nerve Healing

When we identify nerve pain, we use a three-step approach to relieve pressure, calm inflammation, and help the nerve recover:

  1. Spinal Decompression – Gently relieves disc pressure and creates space around the nerve so it can heal.
  2. Deep Tissue Laser Therapy – Stimulates healing at the cellular level, improves circulation, and reduces inflammation around the nerve root.
  3. Neuromuscular Re-education Rehab – Rebuilds strength, coordination, and stability so the pain doesn’t return.

A Patient Story: Laura’s Recovery

Laura came to us with shooting pain down her right leg. She had tried yoga, stretching, and massage — but nothing worked. In fact, some of it made things worse.

When we tested her, we found reduced reflexes and weakness in her ankle — classic signs of sciatica and disc-related nerve involvement..

We started her on decompression, laser therapy, and customized rehab. Within six weeks, her pain was significantly reduced, her strength returned, and most importantly — she regained confidence in her movement.

The Takeaway

Back pain and nerve pain are not the same. If you’ve been stuck treating your pain like it’s just a muscle problem — and it keeps coming back — it might be time for a different approach.

The sooner you identify nerve involvement, the better your chances of recovery.
